Abstract

Medical records are very important for recording health service transactions and must be maintained so they are not damaged. Initial observations at the Kopo Community Health Center showed that 18 out of 30 files (60%) were damaged, which could disrupt health services. This service activity aims to identify and overcome the factors that cause damage to medical records, as well as increase officers' knowledge about maintaining medical records. The methods used are briefing, discussion and question and answer. The results of this activity show that damage to medical records is influenced by various factors, both intrinsic and extrinsic. Intrinsically, some medical records are made from fragile materials such as paper and pink cardboard without folder protection, so they are easily subject to physical damage such as tearing and dust. Meanwhile, extrinsic factors include non-optimal storage environmental conditions, such as exposure to direct sunlight, uncontrolled room humidity, and dust that accumulates around storage shelves. This activity succeeded in increasing the knowledge of community health center officers regarding the factors that can cause damage to medical records and providing practical solutions to prevent further damage. In this way, it is hoped that the quality of medical record management at the Kopo Community Health Center can be improved, thereby supporting the creation of better and more sustainable health services for the community.
