<p><em>This study aims to determine the analysis of the causes of returning outpatient claim files in terms of the requirements for submitting claims at RSU Hermina Arcamanik. The method used is a qualitative research method with a descriptive approach. Data collection techniques used are observation, interviews, literature study, and documentation. Sampling in this study was a portion of the verification results claim files in January-March as many as 83 files. From the results of research conducted regarding the causes of returning claims from outpatients, there are problems including: (1) Incomplete filling in of files submitted to BPJS</em></p> <p><em>(2) Lack of accuracy of officers in coding (3) Often claim files are lost or piled up due to by the absence of a special place to store claims files that have been processed or those that have not been processed. <p>Penelitian ini bertujuan untuk mengetahui perancangan sistem informasi pelaporan kematian dengan menggunakan metode SDLC dan memakai metode pengembangan Advance V Model. Metode penelitian ini dilakukan menggunakan penelitian kualitatif dengan pendekatan deskriptif. Dengan menggunakan metode tersebut, sistem akan lebih mudah beradaptasi, responsif, dan dapat dikembangkan sesuai dengan kebutuhan pengguna. Hasil dari penelitian ini dengan menggunakan metode SDLC dengan pengembangan Advance V Model dalam perancangan sistem pelaporan kematian memberikan manfaat yang signifikan dan dapat membantu tenaga kesehatan agar dapat mengetahui penyebab kematian pasien dengan lebih mudah. Dengan gambaran rancangan sistem <em>Use Case Diagram, Activity Diagram </em>dan <em>Flowchart </em>untuk memvisualisasikan gambaran komponen yang sistematis untuk pembuatan sistem pelaporan kematian. Sistem aplikasi pelaporan kematian ini diuji dengan menggunakan Black Box Testing sehingga hasilnya sesuai dengan yang diharapkan. Masalah yang ada di suatu rumah sakit di Kota Bandung, proses pelaporan kematian dikerjakan secara fisik dan belum dilakukan secara otomatis. Hal ini dapat memperlambat penyajian data laporan kematian dan menyulitkan pembacaan tulisan surat kematian. Selain itu, sertifikat kematian juga terancam rusak ataupun hilang, selain itu pencatatan secara manual akan memakan waktu lebih lama dalam pengerjaannya, sehingga menyebabkan informasi terlambat ditransfer. <p><em>Health service indicators need to be observed, one of which is the efficiency of inpatient services. One way to measure hospital efficiency is to calculate four parameters: BOR, LOS, TOI, and BTO. Then, these parameters can be represented by Barber Johnson graphs. Objective: This study aims to analyze the efficiency level of bed usage at Cililin Regional General Hospital (RSUD) Method: The research used is descriptive quantitative method. The variables used are Average Length Of Stay (ALOS), Bed Occupancy Rate (BOR), Bed Turn Over (BTO) and Turn Over Interval (TOI). The sample used is in the form of recapitulation data from January to December 2022. Results: Based on the results of the study from January to December it was found that January to August were outside the efficient line. One of the reasons was the aftermath of Covid-19. <p><em> </em></p> <p><em>Medical record security based on research from the physical, chemical, and biological aspects. The purpose of this study is to determine the safety of medical records in maintaining the confidentiality of room filling. The research method used is qualitative. The research was conducted at XYZ. The tools used in research are interviews and observasion. Data processing techniques used are non-statistical. The researcher concluded that the security of medical records based opn physical, biological, and chemical aspects can be said to be not optimal, because active medical records are still available on the floor in cardboard box. Confidentiality of medical records is contained in the medical records room which has 2 places. Room 1 already uses a fingerprint door to maintain security and confidentiality, room 2 hasn’t used it so you have to pay more attention because in that room the files are confidential. Confidentiality is also not maximized because the filling room has not used a fingerprint door so there are still other officers who can enter the filling room. Special human resources for strorage are still lacking. <p><em>The lighting for the medical record room at the Wanareja Health Center 1 was uneven throughout the room and the area of the storage room did not meet the standards. This type of research is descriptive qualitative. The object used is the medical record room. The research instruments were room thermometer, meter and lux meter. How to collect data by observation and interviews. For data analysis using descriptive analysis. The results of the study showed that the lighting in the medical record document storage room at the Wanareja Health Center 1 ranged from 137-140 lux but was not evenly distributed throughout the room while the patient registration room ranged from 101-103 lux, the temperature of the medical record room was 23.1</em><em><sup>o</sup></em><em>C, the humidity in the medical record room that is 51% so that includes humidity, storage space area of 4 m² x 2.5 m² x 3 m². This affects the productivity and performance of the officers themselves. Based on the research results, the proposed solutions and suggestions are expected to improve the lighting of the medical record room and increase the area of the room to make it easier for officers to retrieve or return medical record files so that they comply with established standards and it is advisable to provide lighting, temperature and humidity measuring devices.</em></p> <p><em> </em></p> <p><strong><em>Keywords</em></strong><em>: Ergonomics Aspect, Layout, Medical Record Storage.</em></p>Ayu Amelia

<p><em> </em></p> <p><em>The implementation of quality medical records requires quality health workers and in accordance with their competence. A good workforce has a significant influence on the quality of service in the medical record work unit. To achieve quality service, it is important to ensure that the number of workers available is in accordance with the needs so that the service becomes quality. The quality of service in the medical record work unit is greatly influenced by a competent workforce. The quality of this service is related to the workload, so it is important to maintain a balance between workload and the number of staff so that the service becomes quality. Workload refers to the number of tasks and responsibilities given to the workforce, both physically and mentally. Each job is considered a burden for the individual who performs it, and each workforce has a different ability to handle their own workload. Workload can be in the form of physical, mental, or social demands that must be overcome by the workforce. Workload has a significant impact on the efficiency, effectiveness and productivity of the workforce. Therefore, it is important to strike a balance between the amount of manpower available and the workload to be handled within a given service unit. One of the methods used to determine workforce needs is the ABK-Kes (Health Workload Analysis) method, which can indicate the number of workers needed in health facilities based on workload. This method helps simplify the process of placing or readjusting the workforce in an easier and rational way. <p><em>Initial assessment of patients, whether in outpatient, inpatient, or emergency department, is very important to know what the patient needs to start treatment. The purpose of this study was to determine the completeness of emergency department medical assessments at X Hospital in Bogor City using SIMRS Hinai. This study uses descriptive research methods with a quantitative design, by conducting observations and interviews. The population of this study was e-medrec emergency medical assessment at Hospital X Bogor City conducted on May 19 - June 2, 2023. The sample in this study was 322 e-medrec emergency medical assessments. This research instrument is an observation checklist. The results of the percentage of completeness of emergency medical assessment e-medrec at Hospital X Bogor City were 153 e-medrecs that were completely filled with a percentage of 48%, the highest completeness was in the Patient Identification section with a percentage of 100% and there were still incompleteness in the Important Notes, Authentification, Good Records section as many as 169 e-medrecs with a percentage of 52%. The percentage of completeness of emergency medical assessment e-medrec at Hospital X Bogor City is in accordance with SOAP. It can be concluded that the completeness of emergency medical assessments in the application of electronic medical records at Hospital X Bogor City is not yet effective because there are still high incompleteness of emergency medical assessment e-medrec in the Important Notes, Authentification, Good Notes section as many as 169 with a percentage of 52% and not in accordance with SOAP. <p><strong> </strong></p> <p><em>This study aims to describe the procedures for registering outpatient care at the BPJS IGD to support the effectiveness of services at Muhammadiyah Bandung Hospital. The research method used is a qualitative method with a descriptive approach. Data collection techniques use observation, interviews and questionnaires. From the research results obtained, descriptive analysis of the questionnaire. service time for patient registration was 84%. Meanwhile, the service provider style factor is 81%, which means that these two indicators are effective. However, there are several obstacles that arise, including: 1) There are still patients who do not know about the use of BPJS; 2) There are many BPJS patients who do not know about the registration procedure for outpatient emergency care and patients are still not active in seeking information about registration procedures at the IGD at Muhammadiyah Bandung Hospital. <p><em>In medical records, patient report management is very important for hospitals in collecting data if there are new or old patients who have been treated will make it easier to find patient data. The Medical Recorder section in charge will carry out the data processing process by inputting user data, doctor data, diagnosis data, visit type data and polyclinic type data</em>. <em>Furthermore, the medical recorder, namely the reporting section, will get the information to be used as material for making patient data reports, BPJS Health inpatient visit reports. In the process of making reports of BPJS health inpatients at hospitals, there are still shortcomings. The obstacle in the process of making BPJS health inpatient reports is that the medical record file has not been filled in completely so that work becomes hampered and there are often delays in submitting reports. The purpose of this study is to design an electronic-based medical record information system in making BPJS health inpatient reports at hospitals. In this discussion, the research used is using qualitative methods in a descriptive approach. In collecting problems, the authors used observation methods and interviews with medical record officers. Inpatient reporting officers are the subject of research while BPJS health inpatient data is the object of research. <p><em>The purpose of this study is to find out how misfiles that occur in medical records are related to storage at the Mukapayung Health Center. The research was conducted using a qualitative descriptive method, namely analyzing and describing the data and explaining in full the results obtained regarding the occurrence of misfiles. The results showed that there were errors in filling out medical records because of the educational level of the 5 officers, only 2 (40%) had medical records, 3 (60%) did not have medical records. Medical record training of the 5 officers who received training, only 2 (40%) and 3 (60%) did not receive training. Based on the research results in the first quarter of 2023, out of 50 samples of medical record files, 19(38%) files were lost, 36(56%) files were not lost, on February 17 (34%). missed files, 33(66%) missed files, on March 14(28%) missed files 36(56%) missed files. Medical record officers 2 PNS medical record staff and 3 honorary no medical record staff. Elements of the SOP method regarding the management of medical records already exist but are incomplete and there is no specific SOP regarding the storage and return of medical record data. Suggestion Conduct regular training for staff on proper filing procedures and the importance of maintaining accurate medical files. <p><em>Type 2 diabetes mellitus is a disease of hyperglycemia due to decreased insulin secretion by pancreatic beta cells or impaired insulin function. An increase in blood sugar can increase reactive oxygen species (ROS), resulting in oxidative stress. Oxidative stress causes hemolysis of red blood cells, so that in people with type 2 diabetes mellitus hemoglobin levels decrease. The aim of this research is to determine the relationship between hemoglobin levels and blood glucose levels in people with type 2 diabetes mellitus. This research was carried out using quantitative methods using correlation study techniques, which is a study to collect data obtained by measuring and recording the results of checking hemoglobin levels and blood glucose levels in type 2 diabetes mellitus sufferers at the Sumedang Regional General Hospital.
